As a full time Case Manager you will meet one-on-one with individuals and families in need of assistance. You will evaluate each client’s immediate and long-term needs; assessing the client for case management services and eligibility for financial assistance. Together with the client you will develop a case plan to help stabilize the client and create a path to increased self-sufficiency. You will work collaboratively with a top notch multidisciplinary team in a federally qualified health center to assist clients in independent living skills and realistic goal setting; provide support needed to access agencies and collaborate with Social Service agencies to ensure appropriate support for clients especially in housing, substance abuse, public health insurance programs, physical and mental health care, and jail release planning.
In addition to understanding the needs of low income working people and people who are homeless, as Case Manager you will have had experience within a social service setting and demonstrated ability to work with multiple agencies to develop a plan of care. You will have experience which demonstrates your ability to be empathic and compassionate with clients combined within a framework of appropriate boundaries. As expected, you will have strong organizational skills as well as basic computer knowledge and basic financial management.
Located in downtown San Rafael, Ritter Center helps prevent homelessness and improve the health and well-being of individuals and families living in poverty in San Rafael and Marin County. Established in 1980, Ritter Center responds to the needs of Marin County’s low-income residents by providing highly professional services with respect and compassion. Assisting more than 4,000 individuals each year, Ritter Center operates as Marin County’s primary access point for individuals and families in gaining access to social services through it’s own programs and a thorough referral network to other vital services in the community. Ritter Center’s main outcomes are transitioning people into housing, employment, and health, mental health and substance abuse treatment as well as improving people’s day to day living conditions.
